Poor adherence to medication therapy, especially for the treatment of chronic diseases, is a serious public health problem that leads to adverse outcomes for millions of Americans each year.  Older persons are especially vulnerable to adverse outcomes from nonadherence due to the higher prevalence of multiple chronic diseases requiring complex long-term treatment; age-related alterations in pharmacokinetics and pharmacodynamics; and the incidence of cognitive and functional impairment in this population.  This session will provide an overview of medication nonadherence, and describe tools to assist senior care pharmacists identify older patients at risk for medication nonadherence, detect nonadherence, and intervene to improve nonadherent behavior.

The population demographics of adults with intellectual and developmental disabilities (I/DD) are following a trend toward increasing survival with increasing age. Older persons with a developmental disability are at greater risk for age-related chronic diseases and health related functional impairments. Research indicates that older persons with developmental disabilities may exhibit relatively high rates of mobility impairments, fractures and osteoporosis, visual and hearing impairments and mental health conditions, as well as psychotropic drug polypharmacy. This session will help pharmacists meet the unique medical, neurologic, and psychiatric needs of these individuals and understand important social and medical models for promoting health, independence, and quality of life for aging I/DD clients.

How prepared is your pharmacy for natural disasters such as hurricanes, floods, fires, or pandemics?  While most long-term care pharmacies have developed a disaster plan, as required by state regulations and accreditation standards, these plans are often not comprehensive.  Other pharmacies may have only informal plans that have never been set in writing or tested.  This session will highlight practical issues that should be considered when developing plans for both your pharmacy and the customers you serve.
